This is a square metal lens hood from Komura. It features a 55mm screw-in mount and a mechanism where you rotate a ring to tighten and secure it after aligning it horizontally.
It seems that either the mounting technique was poor or too much force was applied, causing the mounting part to detach from the main body when attaching it to the lens (see photo 1).
It appears to have been attached with adhesive, so we recommend repairing (gluing?) it yourself.
