This is an officially licensed NFL item made by American Needle, a long-established hat brand from the United States.
This is a very rare vintage cap from the late 1980s to early 1990s, featuring the old Philadelphia Eagles logo embroidered on it.
It's a double-name specification with the CCM tag, which is difficult to obtain nowadays.
