My dog is a Chihuahua with a rather plump waist of 46cm. I bought size M, but it came off immediately. The velcro was secure though...
Normally, you choose based on waist size, but what's important is the hip size.
After trying various options, I found that size S was the best at not coming off. Although it was for waists up to 43cm, the velcro barely fastened and it fit snugly around the hips!
When choosing girl's diaper covers, it's better to choose one that fits snugly around the hips, even if it's a couple of centimeters short of the waist measurement, rather than worrying about the waist size. The waist can stretch!

Thank you for your consideration m(_ _)m
